Investigation Summary
On February 29, 2000, Employee #1, an experienced worker at a company that did communication wiring for companies around the United States, was installing overhead communication wiring in the 8 ft ceiling of a building. A rack of storage batteries, 45 in. above the ground and 18 in. wide, served as his work platform to access this area. Employee #1 was standing on these batteries and working with both hands above his head when he apparently blacked out or, for some reason, fell backward from the stack of batteries, striking his head on the tile floor. Employee #1 suffered a laceration to the back of his head and internal bleeding in his skull, for which he was hospitalized. The apparent causal factor was that Employee #1 utilized an improper work platform to gain access to the overhead area.
