Investigation Summary

Investigation Nr: 201062270
Event: 08/12/1999
Employee's ankle fractured when crushed by forklft

At approximately 9:30 a.m. on August 12, 1999, an employee was injured while operating a forklift. The employer has been engaged in foundation shaft drilling since 1962. The employee had lifted three rolls of wire all the way up the mast with a one-ton electric forklift. He then turned toward the yard storage area when the forklift shifted to the left and rolled over. The employee fractured his left leg, 3 in. above the ankle. The employer stated that the employee was pushing his left foot on the metal loading ramp to prevent the forklift from tipping over when his left leg was pinned and crushed. The accident was caused by driving and turning while the load was elevated about the center of gravity. There were no witnesses to the accident and no defects were found in the forklift. The employee has five months experience, has been operating the forklift daily, and has been trained.
