Investigation Summary

Investigation Nr: 120269782
Event: 04/23/1999
Employee's arm fractured when struck by steel beam

Employee #1 and coworkers, all experienced and regular employees, were engaged in steel fabrication at a steel yard. Employee #1 was cutting a 8 ft section from a 90 ft by 12 in. steel I-beam, which was supported on two workhorses. Unknown to Employee #1, a coworker had placed two other I- beams on the far end of this beam. When Employee #1 cut off the 8 ft length, the weight of the other I-beams resting on the remaining 82 ft long beam caused it to swing upward and teeter. Employee #1 was standing at the left end of the beam as it swung up, fracturing his right arm. The causal factor for this accident was the coworker placing the additional I-beams on the original beam while it was being cut.
