Employee's finger amputated in packaging machine

At approximately 8:00 a.m. on November 23, 1994, Employee #1, of the Golden State Foods Corp., was operating a W.A. Lane, Inc., L-24 packaging machine, model L2A, serial #2401. He opened the automatic packer unit while it was still running to remove film that had jammed in the machine. When he placed his right hand across the seal area to remove the film, it became caught in the machine and his middle finger was amputated. After the investigation, the employer was cited for a serious violation of T8CCR 4184(a). Other violations were found during the investigation and the employer cited.
