Investigation Summary
At approximately 12:16 p.m. on August 7, 2001, Employee #1, a sales representative and a licensed electrician, was rewiring the paint room's electrical installation to upgrade it to 3-phase system and he was also going to add light fixtures. Employee #1 was standing on an extension ladder and was putting three wires in and pulling one out. Employee #1 lost his balance as he was pulling the wires out and fell onto the concrete floor. The ladder itself did not fall. According to Employee #1, the floor was dry and there were no obstructions on the floor. The extension ladder was tied to the conduit in two places, creating a "Y "like configuration. Employee #1 broke his left ankles and two bones in the right foot, bruised his left knee, left shoulder, and left ribs. Employee #1 was hospitalized.
