Investigation Summary

Investigation Nr: 201055373
Event: 03/16/2001
Employee sustained fracture of ankle when cart tipped over

At 7:20 a.m. on March 16, 2001, Employee #1, who was working in McDonald Douglass Layout room of Building "E," was pushing a wheel cart with a spar tube on it to the extractor machine. Unexpectedly, one of the wheels from the cart caught an air hose laying on the ground, causing the cart to tip over on top of Employee #1. She sustained serious fracture of her right ankle. Employee #1 was transported to Santa Teresa in Duarte hospital for emergency care and was hospitalized. The Division determined that the contributing cause of the accident was the floor area where traffic occurred as a part of normal operation. The floor area was not kept clear from dangerous obstructions which caused tripping and tipping hazards. The company engages in the manufacturing of composite structures for various aircrafts such as main and tail rotor blades for Apache Helicopters, C-17 spoilers, Allison engine fan containment cases, and 767 freighter cargo barriers.
