Finger tip amputated in machinery

At approximately 7:00 p.m. on May 4, 1999, an employee amputated the tip of his right middle finger. Employee #1 was working as a maintenance mechanic lead on the night shift, and had been doing this for approximately ten years. Two other workers noticed that the bottom of the plastic bags were not sealing on the Number 4 form and fill packaging machine and called Employee #1 for repairs. It was determined that the nitrogen flow was too much and needed to be adjusted. Nitrogen forces out the oxygen and keeps the product fresh. The amount of the nitrogen flow is determined by the size of the bag. After the product goes into the bag, the nitrogen is injected into a metal tube and fills the plastic bag prior to the jaws sealing the bag. After the nitrogen was adjusted, Employee #1 noticed the plastic film that forms the bag was not properly positioned on the former. A three-position selector switch, located on the control, can be placed into the filled bag, neutral or empty bag positions. If the control is in the neutral position, the seal jaws slide up and down the plastic, but do not close. In the other 2 positions, the jaws close and heat-seal the bag. Employee #1 thought he put the selector switch in the neutral position, but he actually put the switch in the empty bag position. When Employee #1 attempted to straighten the film that forms the plastic bag, his right middle finger got caught in the sealer jaws and amputated the tip of his right middle finger. The amputation involved bone loss. The two coworkers were in the vicinity but did not actually see the accident.
