Employee's finger crushed in machine

At approximately 11:25 p.m. on May 8, 1995, Employee #1, of Tabco Enterprises, Inc., in Pomosa, CA, was in a compression room cleaning a stock DS-3 machine while it was running. The machine has hazardous moving parts at the back and side of its work table and metal pins/bars that move up and down. Employee #1's right index finger was caught in the moving parts and was partially amputated. He did not disconnect the machine's power supply. During the inspection, the metal pins/bars were found to be unguarded. Employee #1 could not be contacted about the accident.
