EMPLOYEE INJURED WHEN FINGER AMPUTATED BY PACKAGING ROLLER

At 10:45 a.m. on August 4, 1998, Employee #1 was in the process of setting up a vitamin packaging machine by feeding the end of a cellophane sheet into the in-running rollers of the packet sealer on this machine. Her gloved hand got pulled into the rollers and the tip of her left middle finger was amputated before she could free her hand from the machine.
