Employee's arm entangled in unguarded moving sprocket

On February 21, 1998, an employee was repairing a section of a conveyor system when the sleeve of his jacket got caught in an unguarded moving sprocket. His left arm got twisted several times before he could free his arm. He suffered severe damage to his arm, which had to be amputated later in the hospital.
