Employee Arm Crushed by Forklift Mast

On July 27, 1999, Employee #1, a project coordinator, was assisting shipping personnel in loading a large crate into a sea container on a truck trailer. Due to the large size of the container, the crew had run a strap over the top of the crate and hooked it onto the forklift hydraulic cylinder. When the crate was lifted all three tiers of the lifting mechanism went up. Employee #1 reached through the raised mast to release the strap. Once the strap was released the second tier of the mast dropped crushing the left arm of Employee #1.
