Employee's fingers amputated in packaging machine

On June 18, 1999, two employees were in the process of making some adjustments on a sauce packaging machine when plastic packaging material got jammed in the machine. Employee #1 was pulling the jammed material out when Employee #2 activated a knife in the machine by pushing a button on the control panel. The machine actuated and amputated two fingers on Employee #1's right hand. He was treated at Western Medical Center in Santa Ana.
