Employee injures fingers in conveyor roller

Approximately 4:40 a.m. on March 28, 2002, Employee #1 was working inside the employer's facility on west end of the building near gate 28. There was a belt conveyor starting from the west side of the building and extending 20 ft towards the west side. The conveyor structure was supported on a platform and the platform height from the ground was 51 in. The belt width was 34 in., and the powered roller towards the building end of the belt was 8.5 in. in diameter, its rotation being counterclockwise (when facing the south). Across the width of the belt (roller end) there were two 12 in.-high-vertical plates (2 ft in length) located within 2 in. from the two edges of the belt. Just before the accident when the UPS packages were being moved by the conveyor, one of the packages got jammed in the area where the plastic plate developed a sag. When Employee #1 tried to remove the jam, his finger found its way through the sag (which was near the driving roller) and under the belt. The finger moved quickly with the belt towards the nip point of the roller and the finger tip (right hand middle finger) was severed. The index and ring fingers were also injured to a lesser extent. Employee #1 was hospitalized at the Western Hand Center. During surgery Employee #1's finger (middle) bone was affected. Belt conveyor head pulley was not properly guarded making it possible to reach near it and get caught in the nip point.
