Event: 12/23/1993
Employee dies after walking backward into a moving loader

At approximately 2:50 p.m. on December 23, 1993, Employee #1 was working as a flagman for a construction crew that was installing sewer piping along Buchanan Rd., in Pittsburg. Employee #1 was standing at the intersection of Buchanan Rd. and McFaul Dr. and had stopped vehicle traffic on both directions of Buchanan Rd. Employee #1 signaled the front end loader to make a left turn onto Buchanan Rd. As the front end loader was turning left to go westbound on Buchanan Rd., Employee #1 stepped backward into the rear tire of the moving front end loader. He was taken to John Muir Hospital for a serious head injury and a broken pelvis. He died the next day at 10:30 a.m.
