Investigation Summary
While his supervisor was away driving a D7 Caterpillar, Employee #1 saw a tree marked for cutting and decided to fell it while waiting for the supervisor to return. Employee #1 cut the tree so that it would fall up-hill. However, when he put in a backcut on the other side, the tree sat back on the saw. This prevented Employee #1 from finishing the backcut, while preventing the tree from going over. When the supervisor returned, Employee #1 told him about the problem, and asked him to push the tree over with the D7 Caterpillar. The supervisor had to push dirt up so as to build a ramp to reach the tree with the saw blade of the D7. When the supervisor touched the blade to the tree, Employee #1, who had been standing off to the side, returned to the base of the tree and finished the backcut. The tree went over backwards, away from the hillside. At the same time the D7 started going backwards, and Employee #1 fell under the D7 blade, which crushed his leg. He was hospitalized and later had his leg amputated.
