Employee's fingertip crushed by cylinder in vise

Employee #1 was the manager of a planer mill. The rod head of the hydraulic cylinder for the #1 planer had come loose due to stripped threads. To replace the head, the millwrights were removing the head from a new cylinder that was in the millwright shop. The new cylinder was in a vise and a millwright was pulling on the head to pull the rod out. The cylinder tilted in the vise and Employee #1, who was watching, leveled the cylinder. While Employee #1's hand was still on the cylinder, the millwright pulled hard on the head. The cylinder moved in the vise and pinched Employee #1's finger, crushing the tip. The tip was amputated to just below the fingernail. No violations were found in connection with this accident.
