Investigation Summary
On May 12, 2010, Employee #1 was assigned to the dehorning station, the dehorning machine blades were not lining up to make a clean cut. Employee #1 attempted to repair the machine without authorization and without using lock out-tag to deenergize it. Employee #1 had a coworker close the blade, and put his hand into the slide area to check the bolts on the hydraulic cylinder. His coworker let go of the two hand control on the machine opened up smashing his right index finger. Employee #1 sustained an amputation to his right index finger. Employee #1 was taken to the Hospital for treatment.
