Investigation Summary
On September 20, 2005, Employee #1 and coworkers were extracting debris from cyclone air scrubber towers. The other cleaning crew inserted an explosive device into a cyclone tower to break up the debris within it. Employee #1 was on the walkway of another tower near the cyclone mid door of the other tower, which was approximately 50 ft away. The charge did not work right away, but about two or three minutes later. The explosion discharged embers from the tower and into the air, which the wind blew towards Employee #1. Employee #1 was struck by the hot embers and sustained serious burns over 60 to 65 percent of his body. Emergency medical personnel were summoned to the scene of the accident. They arrived and transported him by helicopter to the burn unit of a medical center. At the medical center, he underwent burn trauma procedures and treatment, and was hospitalized for postoperative care.
