Employee's Finger Is Amputated by Forklift Load

On August 17, 2005, Employee #1, a truck driver, while working at a CVS loading facility in Selma, CA, was removing the tie down straps on his load. One of the straps was caught in the product bin and he had his left hand in the strap slot for support. On the other side of his load, a forklift was picking up product bins and did not see Employee #1. As the forklift picked up its load, the load slipped forward and crushed Employee #1's left hand, resulting in the amputation of his finger. He was taken to the hospital for treatment.
