Investigation Summary
Employee #1 and coworkers were making seed mix during a shift that began at 7:00 a.m. They were standing on a grate over a screw conveyor, and Employee #1 was pouring 50 lb bags of seed into the conveyor. Early in the shift, the grate slipped apart and his foot fell into the moving conveyor, which caught his shoe and pulled his foot into the screw. Employee #1 suffered a crushed foot, for which he was hospitalized. The grate had been placed on top of the screw conveyor, but had not been fastened down.
