Employee's fingertips amputated in chain and sprocket

Employee #1 was cleaning up some product that had spilled on a conveyor belt and the floor. While she was cleaning, she noticed some additional product beneath the 12 in. wide conveyor. She attempted to reach this product using a brush, but the bristles were too long to fit in the 2 1/2 to 3 in. high space beneath the conveyor. Employee #1 decided to reach beneath the conveyor belt from the side opposite the chain and sprocket, and to sweep the product out with her hand or with a fox-tail brush that she had been provided. Her right hand came in contact with the chain and sprocket, amputating her fingertips. Employee #1 did not use the fox-tail brush, which was later found sitting on some boxes.
