Employee killed when struck by falling brake bar

Employee #1 was in a well-lit area, performing a routine wire change on a gang wire saw. One of the wire saw spools became hung up on the wire changing apparatus, breaking two of the wires. Employee #1 shut down the machine, but he failed to remove the wire carrier, a 1 in. pipe, from the changer. When he lifted the brake bar to access the wire spool, the bar did not revert to a safe position because the wire carrier was in the way. As Employee #1 bent down to pull the axle pin on the spool, the brake bar fell and struck him on the back of head below the rim of his hard hat. He was able to walk 75 ft to the shot saw building, where he reported his injury and then collapsed. Employee #1 died of the head injuries sustained in this accident.
