Investigation Summary

Investigation Nr: 170717276
Event: 11/28/1993
Employee's foot crushed

Employee #1 was riding on pallets that were attached to the forks of a forklift being driven into a planting field. When the forklift stopped, Employee #1 thought it was going to back up and pick up more pallets. To keep his balance, he put his foot on the side shifter of the forklift mast. The forklift driver did not back up, but side shifted the load without looking to see where Employee #1's body parts were. Employee #1's foot was crushed. He was taken to the hospital for emergency care.
