Investigation Summary
On June 5, 2007, an employee was attaching a narrow medal strap under the windows to the rear of the building. He was working from an extension ladder with a pneumatic nail gun and had to move the ladder, when he could no longer reach the section of the strap to be nailed. Therefore, he reported the desire to nail as much metal strap from the extension ladder as possible before moving it. At the time of the accident, the employee was standing on a rung of the ladder that was approximately nine feet above the surface supporting the ladder, while working to the left of his body at approximately 75 of maximum reach, with his non-dominant hand. In that position, he used the nail gun to fire a nail into a hole in the strap. The employee reported that the nail hit the head of another nail already driven into that spot, and the nail gun recoiled, which caused him to loose his balance and fall from the ladder. He dislocated bones in his foot, which required surgery and medical treatment in a hospital for more than 24 hours.
