Investigation Summary

Investigation Nr: 202489464
Event: 09/15/2009
Employee Fractures Leg in Lumbar Sorter

On September 15, 2009, a sorter operator, discovered that the sorter machine was jammed and it needed to be cleaned to get the sorter operating. The sorter machine was a three story wood sorting machine that sorted the lumber produced in the A-mill and then at the end of the machine the lumber was unitized. The sorter had three levels that were accessed via stairs and you can walk parallel to each level via an elevated walk way. The operator indentified that the sorter was jammed with lumber that was 2-in. by 6-in. by 12-ft long on the second level at section 18 and 19 of the sorter. The operator called on his radio to get help from an employee and his supervisor. The operator walked to the area that was jammed and as he did he saw the employee on the third level and motioned to him that there was a problem on the second level. The operator deenergized, placed his lock on controls, the sorter and began to remove boards from the ground level as he could reach these boards that were hanging down. Shortly, a supervisor and a coworker arrived, placed their lock on controls and began to assist in removing the jammed lumber. After the operator and the supervisor had removed the lumber they removed their locks from the controls and the supervisor directed the operator to start the sorter and continue with production. The operator started the sorter and the employee was still inside of the sorter on the second level straightening and unjamming the boards. The operator heard the employee screaming and turned off the machine. When the sorter began operating, the lumber began to move. The employee was seriously injured when his left leg was caught between moving lumber and a steel structural member of the sorter. The employee was hospitalized for four days with a compound fracture of his left leg.
