Investigation Summary
On April 13, 2007, Employee #1, a foreman painter, had ascended a metal extension ladder to the tile garage roof at the front of a 2-story single family house. He then pulled the ladder up onto the garage roof and positioned the top of it against the second floor exterior wall to begin painting the gutters. The ladder was not secured and, as he began ascending, it moved from under him. He lost his balance and fell onto the tile roof. He then rolled off the roof and fell approximately 12 feet to the cement driveway. He was hospitalized with bruises, contusions, and abrasions.
