Investigation Summary
At 9:30 a.m. on January 5, 2017, an employee was dismantling pallets using a Dismantler Machine containing an unguarded band saw. The employee worked to remove a wooden block when the circulating band saw caught his glove, which were provided by the employer. The employees' s right hand was pulled into the band saw and his right thumb was amputated.
