Investigation Summary
Employee #1 was standing near a sheave that guided metal cable used in the mooring of ships. He was releasing the cable from one of the mooring ropes when his hand was pulled into the nip point between the cable and the sheave. The tip of his left index finger was severed, and the finger had to be amputated between the first and second joints.
