Investigation Summary
Employee #1 was working for Kirk Builders, a sub-contractor, installing Sheetrock and doing taping on an office building remodeling job. He was taping on the third floor and was standing about 4 feet high on a rolling scaffold to reach the ceiling. One of the four adjustable scaffold legs telescoped back into its leg tube, causing Employee #1 to fall to the concrete floor, fracturing his rotator cuff and his right arm near his elbow.
