Investigation Summary
Employee #1, a shop foreman for a manufacturer of molded/curved plywood, was unloading four pallets of alder veneer from a delivery truck parked along a road. Each pallet was held by five steel straps and weighed around 1,000 lb. Using the "White" forklift, Employee #1 lifted four pallets from the truck bed and started tilting it back into position. The top pallet was higher than the mast and started to slide. It fell behind the employee, hitting the back frame of his seat before bouncing off the rear deck of the forklift onto the ground. The back of the seat apparently pushed Employee #1 against the steering wheel, resulting in multiple fractures for which he was hospitalized.
