Investigation Summary
At approximately 9:45 a.m. on July 21, 1994, Employee #1, a seamer mechanic for Tri-Valley Growers, was working on a salt dispensing machine, located at the east side seamer area, K-line filler, in the cookroom. He had removed the plastic enclosure unit to observe the area and was attempting to clean/adjust the unit while it was running. A disc under the dispenser spins with the can wheel on the belt below. As the holes in the spinning disc line up with the holes in the dispenser above, a measured amount of salt is dropped into the can. Employee #1 was pressing down on the plate/block when his hand slipped and his finger went into one of the holes in the bottom of the dispenser. The spinning disc amputated the tip of his left index finger. He was transported to Lodi Memorial Hospital, where he was treated and released.
