Investigation Summary

Investigation Nr: 170354989
Event: 03/31/1994
Employee suffers multiple fractures when struck by forklift

At 9:50 a.m. on March 31, 1994, Employee #1, a truck driver, was setting stickers on the ground for the placement of drywall that was being off loaded. He was run into by a forklift and was hospitalized with a broken left leg, four fractured ribs, and a fractured pelvis. The Gradall lift truck has a blind side on the right when the boom is carried in a horizontal position, as it was at the time of the accident. This may have been a contributing factor.
