Investigation Summary
On July 2, 1996, Employee #1 was asked to operate a cut-off saw after the usual operator left on sick leave. Employee #1's supervisor showed him how to use the saw to cut boards. The supervisor went through four or five boards, each of which had four to six cuts, and then had Employee #1 do some cuts. The supervisor watched him cut for 7 to 10 minutes and was satisfied that Employee #1 was operating the saw correctly. He continued to check on Employee #1 throughout the shift. Fifteen minutes before the end of the shift, Employee #1 put his right hand on top instead of under the material. His right index finger contacted the saw blade and was amputated at the first knuckle.
