Investigation Summary
Employee #1, a utilities technician, was draining oil from chiller #4. Ammonia mixed with oil discharged into a bucket and splashed onto Employee #1. Employee #2 was chased out of the room by ammonia fumes which got to 85 ppm. Employee #1 went to a hospital, where he was treated for chemical burns to his left forearm. Employee #2 was sent to a hospital for observation and released. Employee #1 was wearing a full face respirator and shoulder-length neoprene gloves; however, some of the liquid got inside his left glove. The accident was caused by a plugged discharge line which, when finally opened, created a longer discharge than usual.
