Employee's fingertips amputated in pulley

Employee #1 attempted to dislodge a chain on a lifting hoist when the chain suggenly came free. The weight of the hoisted apparatus pulled on the freed chain, which instantly pulled his right hand into the pulley. Two fingertips on his right hand were amputated.
