Employee's finger amputated by press

Employee #1 was working as an egg carton press operator in the wrapping department of a company that produces recycled paper products, such as egg cartons, paper plates, and other paper products. He reached with his hand under a guard 15 in. into the running press to clear a broken egg carton, although an extension tool was available. The tip of his left middle finger was amputated and he was hospitalized. Employee #1 violated the company lockout program and was reprimanded for this violation.
