Employee's arm injured when caught in rotating shaft

On December 2, 1996, Employee #1, a chipper chaser at a veneer mill, became caught in the chipper belt. His right hand and arm wrapped around an unguarded shaft at the end of the trash return. He sustained injuries that required hospitalization.
