Investigation Summary
Employee #1's employer had been contracted to construct a wood frame building for the Head Start Program. At approximately 7:30 a.m. on January 17, 1996, Employee #1 was on a beam 16 ft 10 in. above a concrete walk area at the perimeter of an open porch area. The rafter between his feet apparently slipped off the lower end of the beam on which he was standing, causing the upper end to pivot up. He was knocked off the beam. Employee #1 sustained the following injuries: double subdural hematomas, bilateral separation of the #1 ribs from the sternum, multiple fractures including most facial bones, upper jaw, nose, and both wrists, a cut lip and forehead, and chipped teeth. He had not been provided fall protection.
