Employees finger amputated in machine

At 10 a.m. on September 19, 2001, Employee #1 was cutting Velcro on an "S-26 Strip Cutter" machine in preparation for it to be placed on goods the employer produces. Employee #1 was working in the Accumold section of the employer's shop. Adhesive on the back of the Velcro regularly causes cut pieces to stick in the machine where they are released from the blade. Employee #1 reported that on the day of the accident cut pieces stuck to the plate and piled up more than usual. Employee #1 continued to use the machine and many cut pieces piled up, some still against the moving blade. Employee #1 then attempted to clear the cut pieces with her left index finger while the machine was operating. Employee #1 placed her finger in the opening created at the point where the blade moved up after cutting. The opening is greater than 1.5 inches and there was no guard surrounding it. Her finger was amputated and she was hospitalized. The employer provides a cane-shaped metal tool to clear Velcro off the plate. Employee #1 was unaware of this tool at the time of the accident.
