Investigation Summary
At approximately 8:28 a.m. on August 13, 2002, Employee #1, working in a warehouse was using a scissor lift, Number 1A130591, to do maintenance on an overhead conveyor. A coworker was operating forklift Number 115 and was clearing a large passageway, when he ran into Employee #1's scissor lift. Employee #1 sustained several fractured ribs and a punctured lung and was hospitalized.
