Investigation Summary
Employee #1 was working on a high pressure test procedure, testing the change in pressure through a filter using 4,800 psi's with helium gas. When the gas was allowed to flow through the tare tube, Employee #1 heard a huge bang when the tare tube blue out. Employee #1 started looking for the pieces of the tube that was blown out, when he noticed a small tare in his shirt above the belt line. Employee #1 realized that he had been injured. Employee #1 was taken to the hospital where he was treated and released. Employee #1 was interviewed and revealed that, during the set-up, a "tare tube" is required to determine the differential flow rate to be applied to the filter during the test. Employee #1 stated that a tare tube was obtained from the cleaning room, and that he used the tube in the test without examining it for the proper wall thickness as he would normally do. The tube Employee #1 used was for a low pressure, and not for the high pressure test performed.
