Employee injured by flying object

On March 16, 1995, Employee #1 was standing next to a green waste pile directing traffic in the employer's yard. The front left tire of an oncoming 950 wheel loader pinched over an unknown object that was flew up and hit Employee #1's safety glasses. The safety glasses' brim cut his left eyebrow. He was taken to the Anaheim Memorial Hospital for treatment and was released the same day. The speed limit in the yard is 5 mph. No violation was noted during the investigation. Employee #1 was wearing proper personal protective equipment, including a hard hat.
