Investigation Summary
At approximately 7:00 a.m. on March 5, 2002, Employee #1, a Production Supervisor, was ripping 2.25-in.-wide pieces of solid mahogany wood from 0.75-in. by 10-in. stock to be used in a counter top. He was using a 10-in. Circular Ripsaw (Model Number 66, Serial Number 0066924) without a guard. After cutting the last piece with a push stick, he reached with his right hand past and over the saw blade (while standing on his toes) to clear the piece from the saw. His feet slipped, he fell, and his left hand made contact with the exposed saw blade. Employee # 1 was hospitalized at Kaiser Hospital for an amputated left middle finger and cuts to other fingers. He had been wearing leather work boots and the floor beneath the table saw had epoxy grip, but was worn away near the saw prior to the accident.
