Employee's finger amputated by divider gate

On November 8, 2002, Employee #1, a machine operator, was working in a yogurt packaging area of Land O Lakes Inc. The employee was at a Kodiak packaging machine that was boxing up 4 oz cups of flavored yogurt. One of the cups went into the "gates" crooked and had flipped on its side. Employee #1 left the operating station and went to the far side of the machine where this cup was and tried to flip it back, right-side-up. He could only get his right hand pinky finger in the cup area, and when he flipped the cup the gates came down and severed the tip of his finger. He was taken to Tulare Hospital for emergency treatment. Employee #1 was hospitalized.
