Employee Pinned By Falling Object

At approximately 1:00 p.m. on January 6, 2003, Employee #1, a foreman, was standing in a trench approximately 5 ft deep. He was using a shovel to grade an area around existing gas and phone lines. When Employee #1 turned away from a section of the trench, a large chunk of material separated from the top of the section. The chunk fell into the trench, pinning Employee #1 against the trench wall. Employee #1 had been working with a backhoe operator at times prior to the incident. However, the backhoe bucket was not in the trench at the time of incident. Employee #1 sustained fatal injuries.
