Investigation Summary
On August 1, 2003, Employee #1 was working as a chipper checker and was being shown by a maintenance mechanic how to do a specific adjustment on the glue nozzle. In order to do this adjustment the machine must be running. The maintenance mechanic had his by-pass key in place and had the by-pass door open. While observing the mechanic, Employee #1 inadvertently placed his finger in an undetermined pinch point and sustained a partial amputation of his right ring finger. Employee #1 was treated for his injury but was not hospitalized.
