Two Employees Are Injured When End Cap Exploded

On November 19, 2004, Employees #1 and #2, welder assistants, were assigned to pressure test an elbow fitting for leaks. The two assistants were trained how to pressure test fittings by their supervisor. As the employees applied air pressure to the fitting, the end cap exploded from the excessive air pressure. The end cap exploded hitting both employees in the forehead. The two employees had not been instructed, nor trained how to use an air regulator. Both employees received deep lacerations to the foreheads but were not hospitalized.
