Employee injured in forklift fall

Employee #1 backed a Walkie forklift (Crown, E type, Model 20WRTL, Serial Number WI30097, maximum 2000 lbs), out of a truck trailer, when the forklift struck the edge of the dock plate. This caused the forklift to slip, tip over, and fall on Employee #1, who was seriously injured. He was hospitalized for fractures.
