Employee falls when unsecured ladder gives way

At approximately 8:05 a.m. on January 8, 2001, Employee #1, a roofer, was attempting to come down from a flat roof using an extension ladder. The ladder was a 28-ft extension ladder that had been secured in place with four rungs protruding above the 17 ft roof line. He was observed adjusting the ladder before he started to step onto it. A worker yelled to him that the top section was no longer properly secured. Apparently Employee #1 did not hear this worker and stepped onto the unsecured ladder as it began to collapse. During his descent, one of his feet caught the ladder and caused him to fall head first and he contacted the ground with his upper back, neck and head. Employee #1 died.
