Employee killed when head crushed in molding machine

Employee #1, the shop supervisor, was sitting under the point of operation of a 110 ton, Promax 4000 horizontal injection molding machine, serial #050595. He was engaged in set-up work to get the machine back into production and, as part of this, he was reinstalling hydraulic lines. The power for the machine was turned off at the panel, but it was not locked out or blocked out in any manner. The machine activated and Employee #1's head became caught between the tube extension, a steel part attached to the bottom of the moveable platen, and the back wall of the metal compartment of the machine. He was killed.
